PRC Baker Places is a non-profit residential treatment agency that has provided the San Francisco community with alternatives to institutional care since 1964.
Our direct care professionals provide counseling and case management services to previously homeless adults living with co-occurring diagnoses of chronic mental illness and substance use disorder.
All agency services are provided from the philosophy of social rehabilitation which integrates teaching daily living skills in a safe and healthy environment.

Position Overview
The Housing Navigator assists clients in the process of locating and obtaining permanent housing by collaborating with San Francisco’s Department of Homelessness and Supportive Housing, and other external stakeholders.
The Housing Navigator at PRC Baker Places plays a critical role in supporting our client’s care management plan while utilizing high touch points of engagement with clients, staying service focused, and understanding the barriers of what our unhoused communities face daily by leading with integrity and compassion within this role.
Reports To : Associate Chief Clinical Director
Primary Duties and Responsibilities
- A strong commitment to social justice, within the intersections of fair housing
- Assess clients for housing eligibility.
- Prepare an Income Strategy Plan with clients by assessing client’s income and available resources.
- Prepare a Housing Benefits Plan with clients and assist them in implementation.
- Identify any social determinants of health needs other than housing and work with client and staff to refer client to internal and external resources.
- Provide at least two workshops a year on accessing and retaining housing.
- Participate in any community organization groups for housing.
- Develop and maintain resource guides for housing.
- Improve referral and referral tracking systems.
- Develop client tracking systems related to housing, referrals, and successes.
- Data entry using agency databases.
- Generate reports as required.
- Effectively manage caseloads, complying with policy and procedures, workflows, and meet documentation requirements.
- Support clients in moving into new housing.
Minimum Qualifications
- 4 years of experience with one-on-one housing related advocacy required.
- Bilingual (English / Spanish) highly desirable.
- DOJ / FBI / CDSS Background Clearance, required.
- Experience working with a wide variety of populations while practicing cultural humility.
- A strong knowledge of issues confronted by and sensitivity to adults with substance abuse, domestic violence, HIV / AIDS, mental health issues, history of trauma and co-occurring diagnoses.
- Knowledge of harm reduction strategies.
- Experience working with government agencies, nonprofit organizations, and community stakeholders.
- Strong organizational skills with attention to detail and accuracy.
- Ability to work independently with minimum supervision and to prioritize and deal with diverse tasks.
- Basic understanding of general computer skills & practices, proficiency in Microsoft Office 360 preferred but not required.
- Ability to work flexible schedules / shifts / areas.
- First Aid and CPR certifications obtained upon hire.
- Must possess and maintain a valid California driver's license.
