Case Manager - Family Services
Description :
- Provide comprehensive case management services to clients and families in a compassionate and professional manner
- Develop and implement case plans in collaboration with clients, parents, and relevant parties, with the goal of reunification or permanency for the child
- Ensure timely documentation of all case activities and maintain physical case files
- Coordinate referrals to services, maintain knowledge of community resources, and monitor service progress
- Attend court activities, individual case staffing, and all required appointments, meetings, trainings, and workshops
- Adhere to all confidentiality rules and comply with safety procedures and practices
Qualifications :
- Bachelor's degree in a Human Services field, preferably Social Work
- Minimum of one year of relevant experience
- Valid driver's license and ability to travel locally and out of the area as needed
- Proficient in Microsoft Office programs Word and Excel
- Demonstrated sensitivity to cultural and socioeconomic characteristics and needs of the service population
