Case Manager - Family Services

Description :

  • Provide comprehensive case management services to clients and families in a compassionate and professional manner
  • Develop and implement case plans in collaboration with clients, parents, and relevant parties, with the goal of reunification or permanency for the child
  • Ensure timely documentation of all case activities and maintain physical case files
  • Coordinate referrals to services, maintain knowledge of community resources, and monitor service progress
  • Attend court activities, individual case staffing, and all required appointments, meetings, trainings, and workshops
  • Adhere to all confidentiality rules and comply with safety procedures and practices

Qualifications :

  • Bachelor's degree in a Human Services field, preferably Social Work
  • Minimum of one year of relevant experience
  • Valid driver's license and ability to travel locally and out of the area as needed
  • Proficient in Microsoft Office programs Word and Excel
  • Demonstrated sensitivity to cultural and socioeconomic characteristics and needs of the service population
