About The Position
Job Title : Registered Nurse I Job Specialty : Home Health Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 17 : 00) Guaranteed Hours : No Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License : Must have a valid nursing license in hand Certifications : CPR, BLS Must-Have : - Experience with HomeCare HomeBase (HCHB) EMR and proficiency in OASIS within the last year- Reliable transportation, a current driver's license, and required liability insurance- Willingness to travel up to 75 miles from the home clinic- Open to on-call rotations and weekend work as required Job Description : Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.- Conduct home visits and participate in periodic office meetings.
- Manage a caseload of 5-8 patients per day.- Complete documentation within 24 hours of patient appointments.- Achieve weekly productivity expectations of 30 points.
- Be open to floating to nearby clinics if patient census is low.- Participate in weekly clinic meetings.- Maintain complete and timely medical records for each patient.
- Efficiently plan travel routes to optimize time and minimize travel between patient homes.Please ensure that you meet all the listed qualifications and requirements before applying for this role.
