Client location and schedule hours :

Antioch / M,W,F 10 am to 12 pm. - Tu,Th, 10 am to 1 pm. Sat and Sun 10am to 2 : 30 pm

Job Responsibilities :

Social History : Client is a Marine Veteran.

Diagnoses : Diabetic, Post 2 spinal surgeries related to L1 & L2 swelling of disc issues, Spinal Stenosis, Left leg blood clot, Mononeuritis of Lower Limb,

Goals and Objectives : Upon arrival, client needs the laundry checked & tended to. He then needs CG to help give him his medicine.

CG then needs to prepare something for him to eat and serve to him. After he eats, client needs to be cleaned up. CG should engage with him throughout the visit and place any items he may need close to him before leaving.

CG is responsible for bringing in any deliveries (groceries or supplies) from the porch and putting away. May assist with exercises as recommended for leg strengthening. Light housekeeping.

ADL's :

Mobility : Client is currently bedridden.

Toileting : Client is completely incontinent. He should be cleaned up on every shift and checked to make sure he has a clean depends prior to shift end.

Observe for skin redness or breakdown and report to him and Dr if needed.

Eating : Client feeds himself, but needs help with heating up any food and needs refrigerator beside his bed restocked periodically.

He will need groceries put away if they are delivered prior to or during CG shifts. Needs drink refillled before leaving shift and throughout.

Bathing : Client gets cleaned up daily after he eats, changing bed pads or linens as needed. He gets a bed bath 2-3x per week and gets his hair washed 1x / week

Communication : Clearly articulates needs / wants

Qualifications :

Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member

Ability to effectively communicate through written and verbal language

Valid Driver's License and adequate auto insurance

Consent to criminal, elder abuse, motor vehicle and sex offender background checks

Consent to drug and alcohol testing
