Community Educator

Job Title : Community Educator Location : Dallas, TX Home Helpers Home Care of Dallas, a trusted leader in providing compassionate in-home care services, is looking to expand its care management team.

We are hiring a Social Worker / Community Educator to serve the Dallas community. At Home Helpers Home Care, we are committed to making a difference in the lives of seniors and their families.

Key Responsibilities : · Build and nurture relationships with local healthcare providers, community organizations, and families to create referral opportunities.

• Identify and develop partnerships with local hospitals, nursing homes, assisted living facilities, doctor’s offices, and other health providers.
• Conduct informative and engaging presentations on in-home care services, dementia care, and Parkinson’s disease care to families and professionals.
• Perform client assessments, create care plans, and coordinate with office staff to ensure timely service delivery. · Attend community events, trade shows, and networking opportunities to promote Home Helpers Home Care services.
• Maintain a CRM system to track and follow up on leads. · Achieve consistent outreach and deliver messaging as per marketing strategies.
• Participate in on-call management and coordination with internal teams. Qualifications : · LMSW or LBSW preferred; however, professionals with backgrounds in Occupational Therapy, Physical Therapy, LPN, or RN will also be considered.
• Experience in the senior care industry is highly desirable. · Exceptional presentation skills and strong organizational abilities.
• Experience with PowerPoint and other presentation software. · Passion for serving the elderly and improving their quality of life.
