Your Daily Activities :

• Customer Service Excellence : Utilize effective communication to deliver outstanding service.

• Regulatory Adherence : Understand and comply with LTC and IHSS regulations.
• Data Analysis : Review case documentation, medical and behavior reports, financial records, and more to support clients' health and well-being.
• Record Keeping : Maintain accurate records of client interactions and tasks across multiple systems.
• Service Planning : Use both technology and in-person methods to develop and implement service plans.
• Compliance : Ensure all records are HIPAA compliant.
• Resource Knowledge : Develop a deep understanding of community resources to support diverse client needs.

Qualifications :

• Education : Bachelor’s degree in human behavioral science or a related field preferred.
• Experience : Relevant combinations of education and experience, or 5 years of verifiable experience, will be considered.
• Requirements : A working vehicle, valid CO driver’s license (unless active Military family), a clean Motor Vehicle Report (no major moving violations or suspensions in the last 3 years) and auto insurance coverage with bodily injury and property damage liability coverage are required.
