Responsible for providing support to clinical SW / Spiritual Care management of cases.
Responsible for evaluating department member’s clinical performance on an annual basis.
Responsibilities :
- Assists in resolving complex clinical situations.
- Acts as a resource for clinical staff
- Attend weekly team meetings
- Evaluates department members clinical performance annually in a field observation. (SW only)
- Participate in Performance Improvement activities as appropriate
- Ensures compliance with policies and procedures, CHAP, NYS standards and Medicare Conditions of Participation.
- Any other duties as assigned.
Qualifications :
- Master’s Degree in SW.
- Licensed by State of New York as an LCSW.
- Knowledge of End-of-Life theory / practice
- Five years supervisory experience.
- Experience with Inter disciplinary Model of Care
