Job Title : Direct Support Professional.
Job Summary : The Direct Support Professional assists individuals with enhancing daily living skills, supports individuals to accomplish program outcomes, and facilitates community
involvement.
Schedule : Every week :
Every week :
Monday : 4 : 00 pm - 10 : 00 pm
Tuesday : 4 : 00 pm - 10 : 00 pm
2nd & 4th weekend of each month :
Friday : 4 : 00 pm - 10 : 00 pm
Saturday : 12 : 00 pm - 6 : 00 pm
Sunday : 3 : 00 pm - 10 : 00 pm
Duties Include :
Must be able to follow safety practices, protocols, and directives per each individual’s
plan and company policies to maintain the individuals’ health and safety.
Provide assistance with daily living skills and community involvement to assist in
furthering the individuals’ level of independence.
Transport individuals to activities.
Assist in hygiene and personal care including toileting.
Complete general housekeeping (dishes, laundry, snow removal, lawn care, etc.)
Implement and document individuals’ program outcomes.
Apply positive behavior techniques.
Successfully pass all required training.
Push, pull, bend, and lift up to 50 pounds.
Must be able to quickly respond to and react to incidents and emergencies.
Basic computer skills (i.e. : email, timesheets, etc)
Adhere to the rules of conduct and employment expectations as set forth in the
employee handbook.

Required Qualifications :
16 years of age.
Valid driver’s license, with a clean driving record.
Ability to pass background checks by the Bureau of Criminal Apprehension and the Department of Human Services.
