Hospice Case Manager Fulton County
We have recently engaged with a very progressive, well run and financially stable Hospice organization in the Fulton County area of Georgia.
They are a nurse-owned and operated organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.
Our client is offering the following benefits :
- Annual Salary : $80,000-$90,000 +
- Health Insurance-Employer Pays % COBRA
- Dental And Vision Insurance Employer Pays % COBRA
- Mileage Reimbursement-$0.48 Per Mile
- PTO-14 Days
- Paid Holidays- 7 Federal
- 401K with 3% Match
Responsibilities :
- The RN Case Manager Will Maintain a Case Load of About 12-14 Patients
- Must Have MatrixCar e Experience
- Patients Per Day-4 on Average
- The RN Case Manager Will be Responsible for Back-up Call
Requirements :
- Registered Nurse in the State of Georgia
- 2-3 year of Hospice or Palliative Care Experience-Preferred
- 2 years of in-Home Patient Care Experience-Required
- Familiarity with EMR System MatrixCare-Preferred
