Summary :
OMC is looking for a Hospital Campus Social Worker to play a crucial role in supporting patients across the hospital, Emergency Department (ED), and outpatient settings. This position involves assessing the psychosocial needs of patients and their families, providing counseling, crisis intervention, discharge planning, and connecting individuals with appropriate community resources. The social worker collaborates with interdisciplinary teams to ensure holistic care, addressing both emotional and practical needs while enhancing the overall patient experience. This role is integral in ensuring that patients receive the necessary support during their care journey, from the ED through to outpatient follow-ups.

Essential Duties & Responsibilities :
Patient Assessment and Care Planning :
- Conduct comprehensive psychosocial assessments of patients to identify emotional, mental health, and social needs.
- Develop individualized care plans in collaboration with interdisciplinary teams, addressing both immediate and long-term patient needs.
Crisis Intervention and Support :
Provide crisis intervention to patients and families in the ED and inpatient settings, helping them navigate acute emotional distress or trauma.Offer emotional support and counseling, helping patients cope with illness, injury, or sudden changes in health status.Discharge Planning :
Collaborate with medical teams to ensure smooth and safe transitions from hospital to home or outpatient care.Coordinate referrals to community resources, home health services, mental health services, and outpatient follow-up care.Address social determinants of health (e.g., housing, transportation, financial barriers) that may impact discharge and recovery.Advocacy and Resource Navigation :
Advocate for patients' rights and needs within the healthcare system, ensuring equitable access to care.Assist patients in accessing community resources, including housing, financial assistance, transportation, and mental health services.Collaboration and Team Support :
Work closely with physicians, CMO, nurses, case managers, and other healthcare professionals to ensure a comprehensive, patient-centered approach to care.Attend multidisciplinary team meetings and case conferences to discuss complex patient cases and contribute to decision-making.Education and Counseling :
Provide education and counseling to patients and families regarding coping strategies, illness management, and available support services.Offer guidance on advance care planning, end-of-life issues, and ethical dilemmas in collaboration with the healthcare team.Documentation and Compliance :
Maintain accurate and up-to-date patient records, documenting assessments, interventions, and outcomes in compliance with hospital policies and regulatory requirements.Ensure all activities meet legal, ethical, and professional standards for social work practice.Community Engagement and Outreach :
Develop and maintain relationships with local community organizations, advocacy groups, and social service agencies to enhance patient care and resource availability.Participate in hospital-wide initiatives to improve patient care and address social determinants of health.Continuous Professional Development :
Stay current with social work practices, hospital policies, and community resources.Participate in ongoing training, workshops, and supervision to maintain licensure and professional competence.Knowledge, Skills, & Abilities :

Mobility :
Ability to move around the hospital campus, including walking between departments, patient rooms, and other areas within the facility.Ability to stand, sit, or walk for extended periods, especially when providing patient support or participating in team meetings.Lifting and Carrying :
Occasionally required to assist with lifting or moving light objects (such as patient charts, medical equipment, or personal belongings) weighing up to 10-20 lbs.Rare instances may require assisting with patient transfers or helping patients with mobility challenges, although this is typically done with assistance from other healthcare staff.Handling Stressful Situations :
Ability to manage emotional and physical stress during high-intensity situations, such as patient crises, trauma cases, or end-of-life discussions.Ability to maintain focus and clear thinking during emergency or urgent care settings, particularly in the ED.Qualifications :
Masters in social work accredited by the Council of Social Work Education.Licensed clinical social worker preferred.2-5 years of related experience.State-specific licensure required.
