Fraser is currently hiring a Designated House Coordinator to join our Community Living team. As a DHC, you will provide site-specific supervision and leadership to assure continuous quality improvement in active treatment service development and implantation at one of our Supervised or Supportive Living locations.

Requirements :

Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.

  • HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing / assisted living home experience
  • BA / BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities
  • Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities

Responsibilities :

  • Manage staff coverage to meet the needs of the clients served
  • Coordinate completion of all household repairs and other upkeep
  • Coordinate the activities of daily living including personal care, hygiene, and grooming
  • Provide direct cares to our clients
  • Assure documentation for all areas of client care are completed
  • Collaborate with medical professionals to coordinate health and medical needs for each client
  • Coordinate weekly, meal planning, and nutrition for clients

  • The starting pay range for this role is $21.50 - $23.50 per hour depending on up qualifications
