Job Description :

As a Home Health LPN, you will play a pivotal role in delivering high quality skilled nursing care to our patients in their homes. You will work closely with the RN Case Manager to ensure the highest level of patient care and achieve optimal outcomes. Your skilled nursing home visit responsibilities will include :

• Administering prescribed medications and treatments as per physician's orders
• Assisting with wound care, catheter care, and other specialized procedures
• Monitoring and documenting patient progress, maintaining accurate and up-to-date records
• Educating patients and their families on health management, medications, and treatment plans
• Collaborating with registered nurses, therapists, and other healthcare professionals to ensure seamless care coordination.
• Communicating effectively with physicians and other members of the healthcare team
• Adhering to all regulatory guidelines and maintaining patient confidentiality
• Monitoring and documenting patient progress
• Compliant and timely charting / documentation of skilled nursing visits
