The Senior Community Care - Patient Care Coordinator is a trained healthcare resource that partners with and supports the Optum Care Team consisting of an Optum Advanced Practice Clinician (APC) and a Registered Nurse (RN). The PCC assists in care coordination activities, administrative tasks and support for members residing in our care setting portfolio. The roles include strong technical skills, communication and collaboration with the clinical team, facility staff and members and their families. The PCC reports directly to the Clinical Advisor.

Schedule : This position is full-time, Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8 : 00am - 5 : 00pm.

We offer 6 - 8 weeks of paid training. The hours of the training will be based on schedule or will be discussed on your first day of employment.

This position is Field Based and requires regular travel to various locations as part of your daily responsibilities.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
