|  Join our team as a Social Services Coordinator II and make an impact by coordinating outside psychiatric and psychosocial services to assist with behavior modification plans, ensuring comprehensive care for residents.

  • You will assess resident needs through social service interviews, provide counseling, advocacy, discharge planning, and access to community resources.
  • As a social services coordinator, you will encourage resident participation in individual and group programs, both inside and outside of the facility.
  • You will facilitate family involvement in care plans and assist residents with legal documents, ensuring seamless transitions and ongoing family support.
  • Additionally, you will meticulously document social service plans and progress notes in resident charts, reflecting goals and interventions effectively.

    • Schedule :

  • This position is full time Monday-Friday, 8am-4pm with a weekend rotation.  What you will need :   Experience in geriatrics preferred Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds preferred Related experience and / or training; or equivalent combination of education and experience. 3 years of social services experience in a Long-Term Care setting preferred Benefits Available :   Health insurance coverage Dental and Vision Plans 403b Savings Plan Daily Pay Paid Time Off Tuition Reimbursement program WellBeing mental health support 24 / 7 Chaplain Counseling 365 Program for new team member development To apply, please complete the required questionnaire.
