job_description.job_card.job_descriptionWork in a home health setting serving the Yakima County area Patient population includes orthopedic, post-op (hips / knees), discharges, deconditioned, fall risk, and post-stroke neuro patients Average caseload of 5 patients per day with both routine visits and evaluations Work days are 10-hour shifts, typically Monday through Friday with optional four 10-hour shifts per week Weekend rotation required, working Saturday every 5-6 weeks with one weekday off during that week No on-call responsibilities Will float as needed throughout Yakima County Uses Epic EMR; Dell laptop and Apple iPhone provided Daily responsibilities include conducting patient visits in home settings and participating in evaluations Support is available from supervising PTs, PTAs, schedulers, and insurance authorization staff A tiered orientation model with skill check-offs is provided, typically completed within the first week depending on skill level Professional attire or scrubs of any color are required; gym clothes are not permitted Team culture is small and supportive, featuring daily morning huddles and a weekly case conference every Tuesday Must be able to work independently while maintaining strong team collaboration Participation in a holiday rotation is expected Requested time off must be approved in advance First-time travelers will not be considered for this position Graduate of an accredited Physical Therapy program Active WA State Physical Therapy license required if applicable Current BLS certification required Minimum 2.5 to 3 years of experience, preferably in home health with a mix of inpatient and outpatient experience highly preferred Ability to lift, carry, bend, twist, and perform general physical therapy duties Experience working with Spanish-speaking patients; must be comfortable utilizing an interpreter Personal vehicle and valid driver’s license required OASIS certification preferred but not required High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
