Your Role as a Home Health Physical Therapist :

In this rewarding position, you’ll :

  • Provide home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury.
  • Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to achieve patient goals.
  • Observe movements, conduct assessments, document progress, and adjust care plans as necessary.
  • Assist patients with exercises and hands-on therapy to manage pain and enhance mobility.
  • Educate patients about their care plans, established goals, exercises, and self-care techniques.
  • Coach family members on the care plan, therapy goals, and safe exercise assistance.

    • What We’re Looking For :

    To thrive in this role, you should have :

  • A degree from an accredited Physical Therapist Program and an active PT license in Michigan.
  • A minimum of 1 year of physical therapy experience, preferably in home healthcare within the last 5 years.
  • Current CPR certification.
  • A solid understanding of state and federal home health regulations.
  • Excellent clinical judgment, strong interpersonal skills, dexterity, and a compassionate approach.
