Job Overview
The Clinical Treatment Coordinator (CTC) will partner with the physician and interdisciplinary team to navigate the patient / family through treatment episode, including assessment, treatment planning, discharge planning and community re-entry. This includes, but is not limited to : managing patient / family expectations, responding to the crisis of hospitalization, supporting the discharge and community re-entry needs and monitoring and managing patient length of stay and avoidable days.
Responsibilities
Demonstrates clinical competence in the treatment planning process.
Coordinates treatment planning and leads management of each case.
Coordinates / facilitates the development of treatment plan by partnering with the physician and collaborates with patient, family and interdisciplinary team.
Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
Implements, reassesses and adjust treatment plan as necessary.
Demonstrates clinical competence to assess individual patient needs.
Completes an interdisciplinary assessment tool with each patient.
Updates interdisciplinary assessments tool each week.
Demonstrates clinical competency to partner with inter-disciplinary team in the treatment planning process.
Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
Implements, reassesses and adjusts treatment plan as necessary.
Communicates frequently and effectively with interdisciplinary team to ensure continuity and coordination of care.
Documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.
Demonstrates competence in developing a clinically relevant transition / discharge plan.
Partners with physician and family to provide leadership to the inter-disciplinary team
Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities :
Utilizing evidence based policies in clinical practice
Attends and participates in learning opportunities about evidence based practice
Membership in hospital or unit based practice council / research council
Evidence based practice project (e.g. policy development or poster presentation)
Journal club, journal article critique and circulation
Literature review
Development of standard / pathway / guideline for patient care
Presentation
Research study
Required Qualifications
Education
Master's degree in psychology, counseling, social work, marriage and family therapy or a related field.
Experience
License / Certification / Registration
Licensed Independent Social Worker (LISW)
Licensed Independent Clinical Social Worker (LICSW)
Licensed Graduate Social Worker (LGSW)
Licensed Psychologist (LP)
Licensed Professional Counselor (LPC)
LPCC Licensing Candidate

Licensed Associate Marriage and Family Therapist (LAMFT) OR
Licensed Marriage and Family Therapist (LMFT)

Compensation Disclaimer
The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
