Where You Will Work :

The Department of Elder Affairs is in the Southwood Office complex in Tallahassee, Florida's Capital City. Duties And Responsibilities :

• The Hope Navigator shall conduct an assessment to identify the customer's immediate needs.
• Provide the customer with referrals to partners that can assist the customer with services to meet their needs.
• Assist customers in identifying and applying for appropriate benefits and services, providing referrals to other state agencies and other resources to support seniors and their families.
• Conduct an extensive customer-centered assessment to identify barriers preventing self-sufficiency.
• Completes customer referrals to community resources based on the needs' assessment, identified barriers and goals set in the care plan.
• Maintain frequent communication with customers, reassess needs, and complete additional referrals and updates to the care plan if needed.
• Identify and maintain a working knowledge of available community resources to meet the individual needs of each senior.
• Document all pertinent information and ensure that all appropriate forms are completed and uploaded into the program data tracking system accurately and timely.
• Build and maintain relationships with respect, trust, sensitivity and confidentiality to customers, coworkers, and community stakeholders.
• Participate in strategic planning and quality improvement processes.
• Incumbents may be required to work before, during, and / or beyond normal work hours or days in the event of an emergency.
• Perform special assignments and other related duties as needed.
• Perform other duties as assigned by the Elder Rights Bureau Chief and the Division Director.
