BAYADA Home Health Care has an immediate opening for a Patient Advocate Manager in our Philadelphia, PA Habilitation Home Care office.

Responsibilities for a Patient Advocate Manager :

• Provide superior customer service and quality home care
• Focus on managing coordination of client services and emergent scheduling issues
• Manage your caseload while proactively growing it
• Build lasting relationships with clients, referral sources, payors and community organizations
• Develop strong, communicative relationships with the team
• Maintain effective fiscal management of your caseload by monitoring metrics and client authorizations
• Managers will partner with Clinical Managers to provide supervision and support to Habilitation Technicians
• In collaboration with the Clinical Manager, Habilitation Manager, Direct Support Professionals (DSP) and other teams to act as an advocate for clients in the Habilitation program
• Participate in Person-Centered Planning processes
• Collaborate with the client, family, Case Managers, DSPs and other teams to implement and evaluate client scheduling needs
• Participate in client ISP meetings
• Maintain ongoing communication with Case Managers / Care Coordinators and other stakeholders
