We're hiring a Clinical Liaison (RN) to support the launch and growth of a new hospital-partnered service line in Philadelphia
You'll act as the face of myLaurel inside the hospital—building trust, identifying eligible patients, and promoting our home-based acute care programs. This is a hybrid clinical-strategic role perfect for a nurse who excels in care coordination, patient education, and stakeholder engagement.
Hospital Engagement – Build strong relationships with care teams, physicians, and hospital leadership
Referral Management – Identify eligible patients and guide them into myLaurel's acute care programs
Patient Education – Clearly explain services and care plans to patients and caregivers
Operational Integration – Seamlessly embed myLaurel into hospital discharge and care workflows
Strategic Growth – Collaborate with internal teams to drive program adoption
Track Results – Monitor key performance metrics including referrals, patient satisfaction, and outcomes
Cross-Team Collaboration – Coordinate with myLaurel's internal care and operations teams
Attend hospital huddles and rounds
Conduct chart reviews and referral assessments
Deliver program education to patients, caregivers, and staff
Track referral data and outcomes
Serve as a clinical ambassador for home-based acute-level care
Active, unrestricted RN license (required)
5+ years of clinical experience ICU or ER experience strongly preferred
Note for Candidates : ICU or ER experience is highly preferred. Candidates without this background will be reviewed on a case-by-case basis.
