Overview :

The Empanelment Coordinator plays a key role in maintaining accurate provider-patient assignments and supporting continuity of care. This position ensures that every patient is appropriately linked to a Primary Care Provider (PCP), reviews documentation for accuracy, and makes informed determinations regarding patient-provider assignments.

Key Responsibilities :

Manage provider panel assignments and maintain accurate records of patient-PCP linkages.

Review documentation and determine the most appropriate Primary Care Provider for each patient.

Process patient reassignments in a timely and accurate manner.

Generate, analyze, and interpret provider reports to support decision-making and operational efficiency.

Draft professional patient correspondence related to provider departures, relocations, or panel changes.

Collaborate with internal teams to ensure data integrity and consistent panel management practices.

Maintain confidentiality and adhere to organizational and compliance standards.

Qualifications :

Strong proficiency in Microsoft Excel, including data management and reporting functions.

Excellent written and verbal communication skills.

High attention to detail with the ability to work independently and manage multiple priorities.

Previous experience in healthcare administration, patient services, or data coordination preferred.
