After Hours On Call Triage Visit RN :    We are looking for someone to be available to be On Call outside of normal business hours to take calls and triage patients, but also be available to make a visit if there is a need to our patients in their home setting. This position would work in both a home care and hospice setting.

Hours :    Full-time |  On Call  |  Be on call generally from 5pm - 8am, Monday - Friday and weekends are from 8am-8am. There will be a block schedule to work 7 days on and 7 days off.
