We are seeking a detail-oriented and dedicated MDS Coordinator (LPN) to join our leadership team in ensuring our residents receive exceptional, person-centered care.
Key Responsibilities
Promote Excellence in Care : Oversee and monitor resident care quality, ensuring all services meet the highest standards.
Comprehensive Resident Assessments : Conduct detailed evaluations and interviews to identify residents’ needs and guide individualized care planning.
Strategic Care Planning : Develop and maintain thorough, goal-oriented care plans focused on improving resident outcomes and quality of life.
Collaborative Leadership : Coordinate and facilitate interdisciplinary team meetings with caregivers, therapists, and families to align on care objectives.
Community Partnership : Build and maintain relationships with community specialists and healthcare partners to optimize resident care.
Team Integration : Participate actively in nursing management meetings to foster open communication and cohesive teamwork.
Education & Training : Lead in-service training sessions on MDS, RAPS, and Triggers to promote staff growth and compliance.
Regulatory Compliance : Ensure strict adherence to the MDS and care planning schedule, particularly for non-Medicare residents.
Qualifications
Licensure : Active LPN license in the State of Florida.
Experience : Minimum of 2 years of experience working in a Skilled Nursing Facility (SNF).
Expertise : In-depth understanding of MDS, Medicare regulations, and the care-planning process.
Skills : Exceptional communication, organization, and leadership abilities.
