General Description : The Program Director of PPH1 & VIP will manage two PSH programs within the MCCH housing portfolio.
Essential Duties and Responsibilities :
- Provide direct supervision to 3 PPH1 Case Management Staff and the VIP Lead Case Manager and Nurse
- Manage budgets for PPH1 / VIP including timely submission of expense reports.
- Consult with Deputy Chief of Housing on clinical, administrative, and programmatic issues
- Attend the Coordinated Entry System meetings; coordinate and track the referral process and ensure timely transitions of referred clients into housing.
- Lead HOC PBV processes including regular meetings with HOC to ensure housing stability for HOC recipients within PPH1
- Oversee quality and quantity of the HIP billing and submit monthly invoice to accounting on time
- Attend all County HIP meetings
- Conduct orientation and training of new program staff
- Provide back-up case management services when necessary
- Represent programs at service provider meetings
- Enter client data into HMIS and Apricot 360 as needed
- Monitor case management input in HMIS, Apricot 360 and all additional client file databases
- Monitor contract deliverables and quality assurance of client case files
- Provide data to the Senior Operations Officer for bi-monthly Board Reports and other contract deliverables
- Act as liaison with Coalition Homes staff to coordinate timely completing rental recertifications
- Participate in monthly meetings with Coalition Homes to review rent master list
- Other duties as assigned
Requirements :
Required Knowledge, Skills, and Abilities :
Bachelor’s degree is preferably in a human services fieldMinimum of one year of supervisory experienceHands-on experience in the social and human services field dealing with diverse populationsKnowledge of principles, methods and procedures of case management, housing first, harm reduction and trauma informed careKnowledge of principles, methods and procedures of addiction, mental illness, and co-occurring diagnosed populationsAbility to negotiate and maintain positive relationships with co-workers and clientsAbility to manage and maintain fiscal accountabilityAbility to comprehend and follow guidelines of grants and contract agreementsKnowledge of word processing and spreadsheet softwareAdvocate for homeless populationMust have a reliable car and transport clients as necessary
