We are seeking a dedicated and driven Care Coordinator to join our Santa Cruz team. This role is ideal for someone who thrives in a Care Manager capacity while also empowering their team to succeed. Care Coordinators are the bridge between clients, community resources, and internal support ensuring no client falls through the cracks.
Key Responsibilities
- Act as primary point of contact for assigned clients, ensuring needs are met and progress is tracked.
- Support populations of focus, including :
Pregnant and postpartum individuals
People experiencing homelessness
Justice-involved individuals
Members with complex health and social needs
- Conduct regular check-ins with clients to assess needs and provide linkage to resources.
- Collaborate with other coordinators, supervisors, and community partners to achieve the best outcomes.
- Document client interactions accurately and in compliance with program standards.
- Balance a caseload while helping team members navigate challenges.
Qualifications
- Must live in Santa Cruz County.
- Strong interpersonal and organizational skills.
- Experience working with vulnerable populations preferred.
- Ability to work independently while supporting team goals.
- Driven, compassionate, and adaptable to client needs.
