Lakeview Village is a continuing care retirement community. We have an opening for a Home Health Case Manager RN to work Full Time.
Lakeview's Home Health Case Manager RN provides comprehensive nursing services to individuals and their families in a community setting, with emphases on the promotion and maintenance of health, early detection, and restoration measures. Provides leadership for Home Health Aides and Staff Nurses to ensure that care is properly and effectively implemented.

Job Duties and Responsibilities
Works closely with the primary care physician to develop and coordinate the plan of care.Provides direct nursing care procedures to residents.Demonstrates a full understanding and effective application of ICD-10 coding for primary and secondary diagnoses.Demonstrates a full understanding and effective application of OASIS assessments. Performs OASIS admissions, transfers, re-certifications, resumptions of care, and discharges.Thoroughly and completely records all symptoms, progress, and treatments in the appropriate charting system to ensure the continuity of care, maintenance of accurate legal documents, and caring, professional services.Notifies the primary care physician of any changes in resident condition immediately.Identifies the educational needs of residents and their families according to the plan of care, provides them with education as needed, in regards to the following : disease process, medical self-care techniques, treatment plans, and individualized plans of care.Ensures that equipment and other necessary items and / or services are made accessible for residents.Ensures that residents' living spaces are maintained in a clean, safe, and orderly manner.Participate in on-call duties as directed by the Home Health Administrator / Director of Nursing.Participates in and supports the objectives of quality improvement programs.Participates in resident care conferences.Attends and participates in in-service educational classes and on-the-job training programs.Communicates with insurance case managers as needed.Maintains proper personal appearance and hygiene according to established procedures for Health Care personnel.Develops and maintains a good working rapport with inter-department personnel, other departments within the facility, and off-campus contacts to ensure that departmental programs can be properly maintained to meet the needs of the facility and residents.Ensures that Residents' Rights to fair and equitable treatment, self-determination, individualityQualifications
Must be a graduate of an accredited School of Nursing.Must possess a current, unrestricted RN State Nursing license in the State of Kansas.Must have current CPR certification.Case Manager Certification is preferred.Must demonstrate a full understanding and effective application of State, Federal, and Medical regulations as they apply to Home Health.Prior Home Health experience and / or Case Management experience is strongly preferred.Must have a reliable method of transportation, and a valid State driver's license.Demonstrates good communications skills, including but not limited to effective reading, writing, speaking, and analytical skills, with a fluent understanding of English.Ability to work independently, prioritize tasks appropriately, and exercise sound judgement where procedures cannot be standardized.Ability to administer and follow straightforward written or verbal instructions.Ability to work overtime, holidays, and weekends as assigned.Demonstrates both precision and accuracy in the completion of duties each day.Possesses an empathetic approach to the elderly.Able to work in a team setting with residents, relatives, staff, and others.IND105
