SUMMARY OF DUTIES AND RESPONSIBILITIES :

This position is responsible for supporting the Residential Program by working in collaboration with direct service staff to resolve housing crisis issues. The expectation for this position is to improve client outcomes by increasing agency skills and knowledge pertinent to multiple barriers related to housing.

DUTIES AND RESPONSIBILITIES :

1) Functions in accordance with the policies of Betty Jean Kerr People’s Health Centers Behavioral Health Division, state and federal regulation, and within his / her professional code of ethics.

2) Participate in the development of initial / master treatment plans (ITPs), ITP (annual) updates and 90-day reviews of ITP’s as needed. Ensures DLA-20s and assessments are completed within a timely manner.

3) Provides individual, group, family and / or other practical supports and interventions, utilizing his / her experience and ability to meet the needs of the client and the goals of treatment, and engages in any other activity that meets those needs and goals.

4) Within limits of experience, educational background, and Center time commitments, presents in public forums educational materials to promote community understanding of mental health and enhance community response to mental health problems.

5) According to Center policies, leads and participates in staffing of persons served to identify problems and establish goals and objectives for treatment and assumes management responsibilities for cases assigned.

6) Participates in emergency services to provide rapid aid in cases of emotional crisis.

7) Provides and / or coordinates a comprehensive follow-up to all clients for whom he / she has been responsible.

8) Provide training and consultation on a group and individual basis for Residential Specialists.

9) Participates in all required meetings.

10) Regularly participates in evaluation of self and assigned staff according to Center policies.

11) Review, in detail, client situations and necessary interventions on both a regular and an as needed basis. Participate in the quality assurance process both in presenting or facilitating the presenting of cases and situations and in giving input in cases and situations.

12) Become involved in direct crisis intervention during regular business hours and on a rotating on-call basis after hours and weekends. Review all critical intervention plans and situations in which plans are implemented; participate in monthly case reviews of critical interventions.

13) Be available for individual interaction with clients regarding house / staff issues and work to resolve concerns within a timely manner.

14) Manage the Supported Community Living Program to ensure costs are paid and records are maintained for enrolled participants.

14) Complete paperwork as required.

15) All other duties as assigned.

SUPERVISORY RESPONSIBILITIES

1) Maintain compliance with licensure, accreditation, and State, Federal, and agency rules, standards, and policies.

2) Coordinate facility policies with supervisor, Administration, and State organizations.

3) Report all critical incidents and staff responses to the supervisor / VP and compliance

4) Coordinate data collection and program evaluations.

5) Interview, screen, select and admit clients to the program.

6) Hires and supervises department-based staff as assigned. Evaluate each staff member according to the performance evaluation guidelines. Facilitate the disciplinary process with supervisees as per guideline when needed. Make recommendations for proper credentialing of each supervisee.

7) Regularly review and monitor written documentation, providing corrective feedback and employee discipline as needed, to assure compliance with documentation requirements. Assist supervisees in identification of referral needs.

8) Accompany workers to see clients at their place of residence at time of initial CS home visit and on a periodic basis to assure worker safety and quality performance.

9) Hold regularly scheduled individual supervisory sessions and team meetings with supervisees, all Community Support Specialist

10) Assist supervisees in the effective management of their time, including assuring that each is within the acceptable range of staff to client ratio and no greater than 30 Community Support clients.

11) Maintain needed house and residential supplies.

12) Ensures maintenance issues are reported and addressed within the sites.

13) Review all travel vouchers and certify that mileage claimed is reasonable and for medically necessary activities.

14) Provide training and assistance in developing and implementing program groups.

15) Is responsible to management needs, such as data collection, program evaluation, development of grant applications, maintenance of contracts for services, etc.

16) Is responsive to all Corporate Compliance and Quality Assurance procedures, cooperates with utilization review procedures, and follows established procedures, rules and regulations of the clinical records department.

17) Be readily available to serve as back up to supervisees when supervisees are unavailable (due to being with other clients, sickness, vacation, etc.) or in need of additional expertise.

QUALIFICATIONS :

Master’s degree in Social Work, Mental Health Counseling, Human Services or a related field and two years of post-master’s experience in a behavioral health setting. Residential experience preferred.

CERTIFICATION OR LICENSE REQUIRED :

Shall successfully complete CPR, First Aid and Medication Administration training. All others as required by the field.
