PositionOverview :

TheStaff Pharmacist under the general supervision of the Director ofPharmacy plays a critical role in ensuring the safe and effectivedelivery of pharmaceutical services within our institution.

Thisposition involves performing a wide range of professional functionsin accordance with the Vermont Board of Pharmacy Rules andRegulations.

The Staff Pharmacist will be responsible fordispensing medications providing drug information overseeingmedication management and ensuring compliance with all relevantregulations and standards.

The role also involves collaboratingwith healthcare professionals to optimize patient care andoutcomes.

KeyResponsibilities :

MedicationDispensing &Verification : Accurately dispense prescribed medications ensuring correct dosagesforms and instructions are provided to patients.

Verifyprescriptions for potential drug interactions contraindications andallergies.

• PatientConsultation : Provide expert advice and consultation to patients and healthcareproviders regarding medication therapy including properadministration potential side effects andinteractions.
• MedicationTherapyManagement : Conduct medication reviews and therapy management to optimizepatient outcomes adjusting prescriptions in collaboration withphysicians asneeded.
• RegulatoryCompliance : Ensure compliance with all state and federal regulations includingVermont Board of Pharmacy Rules and Regulations maintainingaccurate records anddocumentation.
• InventoryManagement : Manage and maintain the pharmacys inventory ensuring theavailability of medications proper storage and adherence to safetystandards.
• CollaborativeCare : Work closely with physicians nurses and other healthcareprofessionals to develop and implement patientspecificpharmaceutical careplans.
• ContinuingEducation &Training : Stay current with developments in the field of pharmacy throughcontinuing education and training and assist in the education andtraining of pharmacy staff andstudents.
• QualityAssurance : Participate in quality assurance initiatives to improve pharmacyservices including medication error prevention and adherence tobestpractices.
• TechnologyUtilization : Utilize pharmacy management software and other relevant technologyto ensure accurate data entry medication tracking andreporting.

Requirements

Education&Qualifications :

• Degree : Bachelor s Degree in Pharmacy from an accredited School ofPharmacy. Advanced degrees or certifications in pharmacy practiceor related fields are highlydesirable.
• Licensure : Active Vermont Pharmacy License in goodstanding.
• Skills : Proficient in computer use and data entry with strong oral andwritten communication skills. Ability to effectively interact withpatients healthcare providers and teammembers.

Experience :

• MinimumRequirement : At least one year of professional experience in a pharmacy settingpreferably within a hospital or other healthcareinstitution.
• Preferred : Experience in an institutional or clinical pharmacy setting with astrong understanding of regulatory compliance patient care andinterdisciplinarycollaboration.

Knowledge of State and Federal regulations in Nursing Homes ispreferred. The employee must have the ability to understand,communicate and react effectively to the unique needs of long-termcare patients and families.
