Women’s Wellness Connection Patient Navigator
As a grantee of the Colorado Department of Public Health and Environment (CDPHE), Salud is able to offer the Women’s Wellness Connection (WWC) Program to eligible patients for cancer prevention and early detection.
The WWC Patient Navigator functions as a member of the patient care team performing patient navigation for both insured and uninsured women through the Health Navigation and Clinical Services component of the WWC Program.
The WWC Patient Navigator approaches his / her tasks in a team-based care fashion and provides support and guidance to patients throughout the breast and / or cervical cancer screening process.

- Starting Wage - $ to $ per hour
- Monday to Friday 8 am to 5 pm or 7 am to 4 pm

Qualifications
Education :
High school diploma or GED, required
Experience :
- One to two years of clinical work along with working with a diverse and / or underserved population in a health setting is preferred
- Experience providing patient health education is preferred Licensure :
- None
- None
Knowledge, Skills, and Abilities :
- Able to relate well to patients and co-workers
- Possess sensitivity toward the needs of others
- Must be a self-motivated, proactive, and independent worker
- Possess excellent organizational skills
- Must work well with the patient care team
- Bilingual English / Spanish is required
- Sensitivity to low-income and ethnic minority communities is a must
