Open Door is seeking a Manager, Youth Treatment & Recovery Program located out of our Tarrytown, NY headquarters. The Manager, Youth Treatment & Recovery Program provides day-to-day leadership of the Family Tree Program, an initiative to enhance comprehensive treatment and recovery services for vulnerable adolescents and transitional aged youth with substance use disorders (SUD) and / or co-occurring mental health disorders (COD).
The Manager, Youth Treatment & Recovery Program is responsible for daily oversight of the Family Tree Program, including program management, staff supervision and collaboration with internal and external stakeholders.
The Manager, Youth Treatment & Recovery Program participates in a patient-focused team-based approach to health care that ensures that youth are effectively screened for risks and barriers (social determinants of health), engaged in services and supported as they work to achieve their care plan goals.
Responsibilities
PROGRAM MANAGEMENT
- Coordinates with supervisor and Project Director to develop, implement, and sustain the Family Tree Strategic Plan
- Monitors program performance to ensure access to effective, high-quality care for populations of focus
- Oversees the day-to-day operations and processes of the Family Tree Program including screening, assessment, care planning, and care coordination
- Develops and implements tools, criteria, workflows and engagement strategies to support patients and their families
- Supervises the Youth Treatment & Recovery Program Specialist and Care Managers
- Establishes and monitors referral systems, ensuring successful linkage to services and completion of necessary documentation and data tracking
- Ensures accurate data collection, analysis and reporting; as part of project team, reviews data to assess performance and supports responsive action plans as needed
- Convenes and facilitates team and program update meetings for Open Door leadership; plans agendas, ensures documentation of meeting activities and follows-up as needed
- Supports patient / public education initiatives; collaborates on the development and dissemination of educational information
COLLABORATION AND COORDINATION
- Collaborates with Open Door departments to ensure effective communication and service implementation
- Develops and maintains relationships with key community service providers, medical providers and local coalitions with particular attention to those serving transitional aged youth;
coordinates with community partners to ensure effective, efficient collaboration
- Ensures that program staff have access to, and understanding of, available local resources
- Represents Open Door at internal and external meetings / workgroups
COMMUNICATION
- Informs supervisor promptly in any event that may affect quality and standards of patient or that impact the success of the Family Tree Program
- Demonstrates the ability to function in a culturally sensitive manner within Open Door’s philosophy and mission
- Actively promotes planned care concepts within Open Door
PLANNING
- Attends staff and other organizational meetings, as designated, to ascertain Open Door’s short- and long-range plans, programs, and goals within areas of responsibility
- Participates in staff development programs, educational activities, seminars, and training opportunities
COMPLIANCE MANAGEMENT
- Adheres to organizational and federal requirements regarding data and confidentiality, electronic transmission protocols, and security and maintains patient confidentiality at all times per Open Door policies and procedures
- Adheres to National Patient Safety standards as defined by the Joint Commission on Accreditation of Healthcare Organization and the Open Door Family Medical Centers
- Prepares and maintains documentation in compliance with Joint Commission standards and applicable city, state, and federal laws
- Meets annual mandatory training requirements
Qualifications
EDUCATION
Master’s Degree in Public Health, Public Administration, Social Work or a related field OR a Bachelor’s Degree in one of the aforementioned or related fields with 7+ years of progressive relevant experience AND Credentialed Alcoholism and Substance Abuse Counselor (CASAC), CASAC-Trainee or CASAC-Provisional with direct experience working with people with substance use and co-occurring disorders
EXPERIENCE
- Working with people with substance use and co-occurring disorders
- Working with young people (age 25 and younger)
- Staff management experience
- Program management experience
- Experience working within community / health / social service organizations
- Experience with case management / care coordination and health education
- Experience with electronic medical records is preferred

- Fluency in English and Spanish preferred
