Speech Therapist Home Visits, Per Diem
Atrio Home Health Grand Rapids is seeking to add a per diem Speech Language Pathologist to our team! This home health Speech Language Pathologist works with assigned clients in their homes to assess and treat speech, language, swallowing, and cognitive disorders.

Responsibilities
Performs speech therapy evaluations to assess client progress and recommends care plan adjustments
Diagnoses, administers, and interprets speech, language, swallowing, voice, and cognitive disorders using various tests
Completes OASIS discharges and recertification paperwork per policy
Develops a treatment plan for clients in consultation with physicians and care team
Coordinates services in consultation / communication with other Atrio Home Health staff
Provides speech therapy treatment to assigned clients.
Identifies barriers to desired outcomes and evaluates care results
Informs physicians and other personnel of changes in the client’s needs
Educates clients and their families on communicative, cognitive and swallowing functioning and therapeutic techniques
Functions as a member of the interdisciplinary team attending case management meetings and maintaining communication with other disciplines to facilitate quality client care
Prepares clinical documentation, progress notes, and other required documentation
Qualifications
Speech Language Pathologist License in Michigan
Valid Michigan driver's license, proof of auto insurance, and reliable transportation
2 years of experience preferred; preferably working with the geriatric population
Passion for delivering high quality care to older adults
