MC : Rehab Core IP (Full-Time, Day Shift)-

The Manager-Case Management Rehab is responsible for human resource management of all program coordinators and support staff.
Manages the daily operations of program coordinator, social worker, and case management activities within rehabilitation.
Assists the Executive Director with ensuring Rehab Case Management clinical practice meets expected excellence standards and follows all LLUMC policies and guidelines.
Identifies patients with complex discharge needs and proactively works to address issues. Ensures accurate and timely completion of all Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) and CMS paperwork for assigned teams.
Oversees the management and program development of assigned patient programs. Maintains ongoing communication between team members, families, and the third-party payers to ensure ongoing authorization for services.
Documents case management activities and utilization review as per requirements. These services may be provided for both the LLUMC, LLUCH, and ECH hospitals.
Performs other duties as needed.
Bachelor's Degree in Nursing, Physical Therapy, Occupational Therapy, Social Work, or Speech Therapy required. Minimum three years of hospital and rehabilitation experience in specialty area required.
Minimum two years of Case management experience required. Minimum one year of supervisory experience preferred. CCS paneling will be required following one of year employment.
Licensed Clinical Social Worker (LCSW), California License in Physical Therapy, Occupational Therapy, Social Work, Speech Therapy, or Nursing required.
Case management certification preferred.
Able to communicate positively with team members, physicians, referrals, case managers and colleagues. Leadership skills and organizational skills are necessary.
- Ability to organize and analyze data. Able to acquire new data quickly. Able to : read; write legibly; speak in English (and Spanish preferred) with professional quality;
- use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others;

- Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone;
- identify and distinguish various sounds associated with the work place / patient care; see adequately to read computer screens, medical records, and written documents necessary to position;
discern temperature variances through touch.
