Under the direction of the Program Manager II Patient & Community Wellness, the Program Coordinator will implement all initiative deliverables in collaboration with program staff.
The Program Coordinator will be responsible for coordinating and implementing the Food As Medicine (FAM) program which includes : facilitating food and nutrition insecurity trainings with providers and clinical staff, referring patients to the partner Farmer’s Market, assisting in other FAM interventions, support key Patient & Community Wellness staff, and participate in meetings with NEVHC’s Enhanced Care Management team to support CalAIMS in expanding Food As Medicine approaches.
The Program Coordinator will also collaborate with NEVHC’s Quality Improvement team to monitor the impact of the program.

REPORTS TO : Program Manager II Patient & Community Wellness
To perform effectively in this position, the employee must have :
1. Bachelor’s degree in Public Health, Health Education, Nutrition, Kinesiology, or related field is required.
2. Must maintain a current Cardio-Pulmonary Resuscitation (CPR) certification, in compliance with corporate CPR policy.
3. Good interpersonal skills and ability to present in front of a group.
4. Computer skills in Microsoft Office programs (Word, Excel, etc.), and database systems.
5. Ability to be flexible and work in a changing environment.
6. Available to work weekends and evenings.
7. Sensitivity to the different cultures represented among members and staff.
8. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.
9. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision.
Self-starter, reliable and dependable.
10. Fluency in English (speaks, read and write) is required.
11. Fluency in Spanish (speaks, translate, read and write) is required.
12. Ability to maintain absolute confidentiality about health care and other patient / client information.
13. Current CPR / First Aid, California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
