We are seeking a full time HCS Program Coordinator / Case Manager for our HCS Department
BASIC FUNCTION :
Conducts analysis and assessment of HCS programs, processes and outcomes utilizing quantitative tools and methods to identify performance issues and interventions to be utilized throughout HCS.
Develops, analyzes and manages Health Care Services (HCS) department outcome projects to ensure contractual compliance is maintained.
Produces outcome and program reports and makes recommendations for improvement. Develops and maintains department Quality Improvement Projects.
Key Responsibility Areas :
Provides project support to the directors of HCS programs with the development, implementation, and monitoring of department projects.

Assesses efficiency of current department processes and provides recommendations regarding process improvement opportunities.
Monitors program outcomes quarterly including, but not limited to, Case Management, Disease Management, etc. Validates data, and creates reports to demonstrate HCS program impact to HCS leadership and external stakeholders.
Writes and produces professional quality internal and external position papers, reports and plans. Produces, validates, and delivers accurate monthly dashboards containing HCS program data and ad hoc reports.
Develops and delivers in-services and presentations as needed.
Serves as the point of contact for the HCS department to coordinate efforts such as development of reports, data validation and quality assurance, and improvements to the medical management system.
Participates in internal committees as related to outcome integration.
Conducts research on industry best practices related to program implementation and monitoring to guide department strategies and initiatives.
Designs and conducts new quality studies, coordinates the study with Clinical Quality Management staff and other program staff, continually maintains at least two Quality Improvement Projects (QIPs) for each area assigned, and presents monthly QIP updates at the HCS study committee and staff meetings
Working Conditions :
Works within a standard office environment, with some travel required
Extensive computer work with long periods of sitting
Requirements
QUALIFICATIONS
Education & Experience : Required
Bachelor degree in Social Work, Health Care Administration, Public Health, Business, Statistics, or related field
3+ years experience in HCS program development, evaluation, and reporting
2+ years experience with data management, statistical analysis, report design and presentation
Preferred
Masters degree in Social Work, Health Care Administration, Public Health, Business, or Statistics
Experience with project management, research, program development and evaluation, reporting, business and operational planning, and process improvement
Knowledge & Skills :
Technical : Proficient in Excel, Word, PowerPoint and Visio; Expertise in preparing written communications and presentations;
Knowledge of performance measurement and quality improvement tools and disease management concepts
Problem Solving / Analysis : Ability to solve problems through systematic analysis of processes with sound judgment; Has a realistic understanding of relevant issues
