Job Description :
Under the direction and supervision of the Community Health Services Program Manager, conducts health education, training, workshops, and health promotion activities within PHR 6 / 5S, focusing primarily on counties with more vulnerable populations.
Develops and distributes DSHS-approved culturally and linguistically appropriate content for educational materials. Fosters and builds sustainable and meaningful relationships with community partners and stakeholders.
Assists PHR 6 / 5S tobacco coordinator with educational tools and presentations, focusing on tobacco, nicotine delivery systems (ENDS).
Escalates issues, as appropriate, to management for resolution. Works under general supervision, with moderate latitude for the use of initiative and independent judgment. Essential Job Functions :
40% Conducts health education, training, workshops, and health promotion activities within PHR 6 / 5S, focusing primarily on counties with vulnerable populations.
Plan and implement community outreach activities to promote health education, risk reduction, healthy behaviors and to improve health literacy.
Apply appropriate behavioral theories and evidence-based models to plan, organize, implement, and evaluate community engagement, health education and health promotion programming in community-based settings to increase healthy knowledge and skills in at-risk communities.
Establish and maintain close working relationships with internal DSHS staff, to coordinate and perform culturally and linguistically appropriate health promotion activities.
Escalates issues, as appropriate, to management for resolution.
25% Fosters and builds sustainable and meaningful relationships with community partners and stakeholders. Identify and engage with new and existing community partners to address vulnerable populations.
Inform community of services available and other community resources. Communicate health promotion, health literacy, empowerment, recovery, and resiliency information to communities.
Collect, compile and report on community feedback and / or disparities that impact health.
15% Develops and distributes DSHS-approved culturally and linguistically appropriate content for educational materials, visual aids, presentations, interactive demonstrations, virtual group classes and social media settings.
Content includes, but is not limited to, vaccine preventable diseases, injury prevention, management of underlying chronic health conditions, and pandemic preparedness.
15% Assists PHR 6 / 5S tobacco coordinator with educational tools and presentations, focusing on tobacco, nicotine delivery systems (ENDS) and other relevant topics.
Provide resources, tools and education using evidence-based programs for prevention and cessation. Provide consultative and advisory assistance to community and business leaders, agencies and coalitions, health care providers, educators, and retail merchants in the development of policies, procedures, and strategies to prevent tobacco use and the use of ENDS.
Distribute educational materials to high-risk populations. Identify and share relevant data to support prevention efforts.
5% Performs other duties as assigned, which include, but are not limited to, actively participating and / or serving in a supporting role to meet the agency’s obligations for disaster response and / or recovery or Continuity of Operations (COOP) activation.
Such participation may require an alternate shift pattern assignment and / or location. Knowledge Skills Abilities :
