Shift : 3 PM 11 PM
Salary : $35 per hour

Summary :
The ideal LPN candidate will oversee coordinate and directly address healthrelated concerns of program participants. This role requires a strong sense of responsibility dedication to patient care and the ability to travel to different locations throughout their shift.
Essential Duties and Functions :
- Maintain accurate and current nursing documentation and medication administration records.
- Educate and assist residents with medication management and adherence to physician orders.
- Provide various nursing care services including but not limited to wound care trach care and diabetic counseling.
- Assess the health and emotional needs of residents and adjust care plans accordingly.
- Communicate efficiently with the nursing team program staff and management regarding resident concerns.
- Uphold the highest level of confidentiality in all aspects of the job.
Credentials Required :
- Valid Licensed Practical Nurse (LPN) license.
- Pennsylvania Criminal Clearance (obtained within the past year).
- Current CPR certification.
- Proof of Covid19 Vaccination.
- Valid drivers license.
- Recent Physical and TB or Quantiferon test (within the last 6 months).

Qualifications :
- Demonstrated experience as an LPN preferably in a residential or community health setting.
- Exceptional assessment organization and problemsolving skills.
- Ability to operate independently and as part of a multidisciplinary team.
- Effective communication skills with a strong emphasis on confidentiality.
- Must possess reliable transportation for travel between residential locations.
