Job Title : Licensed Practical Nurse (LPN) Developmental Disabilities Residential Care

Location : Multiple residences within the Philadelphia area

Shift : 3 PM 11 PM

Salary : $35 per hour

Type : Temp to Perm W2 position with full benefits

Summary :

The ideal LPN candidate will oversee coordinate and directly address healthrelated concerns of program participants. This role requires a strong sense of responsibility dedication to patient care and the ability to travel to different locations throughout their shift.

Essential Duties and Functions :

• Maintain accurate and current nursing documentation and medication administration records.
• Educate and assist residents with medication management and adherence to physician orders.
• Provide various nursing care services including but not limited to wound care trach care and diabetic counseling.
• Assess the health and emotional needs of residents and adjust care plans accordingly.
• Communicate efficiently with the nursing team program staff and management regarding resident concerns.
• Uphold the highest level of confidentiality in all aspects of the job.

Credentials Required :

• Valid Licensed Practical Nurse (LPN) license.
• Pennsylvania Criminal Clearance (obtained within the past year).
• Current CPR certification.
• Proof of Covid19 Vaccination.
• Valid drivers license.
• Recent Physical and TB or Quantiferon test (within the last 6 months).

Qualifications :

• Demonstrated experience as an LPN preferably in a residential or community health setting.
• Exceptional assessment organization and problemsolving skills.
• Ability to operate independently and as part of a multidisciplinary team.
• Effective communication skills with a strong emphasis on confidentiality.
• Must possess reliable transportation for travel between residential locations.
