JOBSUMMARY / PURPOSE
Assessesplans implements coordinates monitors and evaluates options andservices to meet an individuals behavioral health needs.
Appliesknowledge of applicable regulations identifies resources (internalor community partners) and eliminates barriers where possible.
Provides care management interventions and seamless support acrosstransitions of care. Works with cases that have a low to moderatelevel of service intensity.
EXPERIENCE / EDUCATION / QUALIFICATION
Experience :
- One (1) year of clinical experience required.
- Experience or training in the followingrequired :
- Healthcare delivery systems and / ormanaged care patients.
- Computer applicationsincluding electronic documentation (e.g. MS Office EPIC ClinicalCare Advanced).
- Experience in the followingpreferred :
- Case management.
- Medicare and Medicaid rules and regulations and healthplan benefit structure and policy.
Education :
A Masters degree in Mental Health Social Work ClinicalPsychology or a closely related field is required.
Licenses Certifications and / orRegistration :
- Currentunencumbered Oregon credential / licensure in one (1) of thefollowing required :
- Licensed Clinical SocialWorker (LCSW).
- Licensed Professional Counselor(LPC).
- Licensed Psychologist (PhD orPsyD).
- Clinical Social Work Associate(CSWA).
- Licensed Master of Social Work(LMSW).
- Licensed Marriage and Family Therapist(LMFT).
Knowledge / Skills / Abilities
Communication Effective written and verbalcommunication skills are provided to members providers andorganization staff.
Able to tactfully discuss issues and listen toand understand complex information / situations. Ability to applymotivational interview techniques when working with members.
Proactively resolve conflicts positively and constructively.
- Confidentiality Knowledge of State andFederal (HIPAA) laws about the confidentiality of protected healthinformation. Ability to comply with laws and maintainconfidentiality of patient information.
- Critical Thinking Identifies complex problems. Involveskey parties gathers pertinent data and considers various options inthe decisionmaking process.
Develops evaluates and implementseffective solutions in a membercentric manner.
Social Perceptiveness Ability to work with a diversepopulation from pediatrics to geriatrics. Basic understanding ofagerelated differences in caring for and / or communicating withmembers and caregivers.
Possess personal sensitivity to the needsand experiences of others and a nonjudgmental attitude towardspersons of differing standards values lifestyles andages.
Knowledge of Medicare and Medicaid rulesand regulations and health plan benefit structure and policy.Ability to appropriately apply evidencebased clinical practice andcriteria to authorization requests.
This is a hybrid position in which people wouldneed to reside somewhat locally in Corvallis OR or would need torelocate.
The hybrid piece is primarily remote and onsite presencein the office member homes or a clinic. This is a nonleadershiprole
