TAS - Case Manager

Position is eligible for a $1250 sign-on bonus!

Transitional Apartment Services Program (TAS) is looking for the right individual to join the team as a case manager! Bring your positive energy to assist people on their journey in becoming independent!

Responsibilities :

• Provide supportive services to program consumers with mental illness with compassion and dignity.
• Maintain charts and documents within the guidelines of Office of Mental Health. Assist with daily living skills of the consumers.
• Assist with light to moderate cleaning as needed.
• Communicate with providers on a regular basis.
• Promote an environment of inclusion and belonging.

Requirements : High School Diploma or equivalent and one-year qualifying experience. Experience working with multiple populations preferred.

Must have reliable transportation and a valid NYS driver's license. Bi-Lingual candidates always welcome.
