Responsibilities

• Implementationof physician prescribed plan of care
• Patientassessments and coordination ofcare
• Administration of prescribed medicationtreatments and therapies
• Compliant timelydocumentation of care provided in system ofrecord
• Health promotion teaching and trainingof family members
• Responsible for accepting andwriting verbal orders from physicians as required and for obtainingcosignature of Case Manager on all verbalorders
• Follow up with physician client familymembers staff and Case Manager on client problems concerns and / orchanges in client condition
• Performs otherappropriate duties as assigned
• Assist patientswith travel to and fromschool

Qualifications

• Musthave and maintain an active unencumbered license (RN / LPN / LVN) inthe state in which the clinician willpractice
• Current CPR certification fromAmerican Red Cross American Heart Association orBLS
• TB skin test (current within last 12months)
• Six months prior handson nursingexperience preferred but not required
• Must havereliable transportation
• Fingerprinting may berequired
• Benefiteligibility is dependent upon employmentstatus
