Position Summary
The 2nd Shift Lead Caregiver is responsible for cultivating a positive work atmosphere by offering training, guidance, feedback, and support to team members. This role serves as both a mentor and role model, equipping staff with the tools they need to grow professionally while ensuring residents receive the highest quality of care at Harvest Home Assisted Living. In addition, the Lead Caregiver will complete the PM medication pass and support caregivers on the floor with direct resident care.
Responsibilities
Demonstrate Harvest Home’s mission and philosophy in all daily tasks.
Provide leadership and support to caregiving staff during the PM shift.
Complete the evening medication pass in accordance with state regulations and company policies.
Assist caregivers with resident care needs, including activities of daily living (ADLs), redirection, and engagement.
Serve as the on-shift point of contact for staff questions, resident concerns, and family needs.
Ensure resident care plans are followed and update documentation as required.
Communicate with the leadership team regarding resident changes, incidents, or staffing concerns.
Promote a positive, team-oriented environment focused on quality care and resident dignity.
Assist with training and mentoring new caregivers.
Perform safety rounds and ensure a clean and organized environment.
Provide leadership, guidance, and support to caregivers throughout the shift.
Address resident care concerns and staff protocol issues in collaboration with the nursing and administrative team.
Offer ongoing training, coaching, and mentoring to team members as needed.
Ensure proper implementation of Harvest Home protocols, documentation, and tools to improve efficiency.
Collaborate with the interdisciplinary team to promote high-quality care and adherence to procedures.
Support administrative staff to ensure effective daily operations.
Participate in scheduled house meetings, mentor meetings, and in-services.
Lead by example to motivate the team toward positive outcomes.
Communicate medication refill needs, and maintain accurate documentation.
Maintain stocked first aid bins and monitor resident care trends (e.g., shower refusals, medication refusals) for follow-up with nursing.
Manage and restock incontinence products in resident apartments.
Address and report complaints or concerns promptly to nursing staff.
Perform all other duties as assigned.
Qualifications :
Prior experience in caregiving, preferably in a memory care or assisted living setting.
Current Medication Administration certification.
Strong leadership and communication skills.
Compassionate, patient, and committed to supporting residents with dementia and their families.
Ability to work effectively as part of a team and independently.
Must meet state requirements for caregiver training and background checks.
Strong problem-solving skills with the ability to create effective solutions.
Excellent interpersonal and communication skills to collaborate with residents, families, and staff.
Effective time management and organizational skills.
Demonstrated integrity, professionalism, and good character.
Must meet all general staffing requirements, including :
Free of communicable disease.
Ability to physically and mentally perform job tasks.
General understanding of the geriatric population and dementia care.
Monday - Friday 2pm-10pm and every other Holiday
