Woodfords Family Services has openings for full-time (40 hours per week) Case Managers within our Therapeutic Foster Care Program.
These positions include a highly relational element that include emphasis on developing healthy, meaningful relationships with foster families.
For this reason, the TFC Case Manager has a well-balanced and manageable case load.
We are currently seeking qualified individuals in the following areas : Westbrook, Bath, Lewiston, Manchester and Livermore.
These positions are eligible for hybrid and remote work options!
Position Summary
Woodfords' Therapeutic Foster Care Program (TFC) provides treatment-level foster care to children 3-21 years with developmental and / or emotional disabilities who cannot reside with their biological family.
With the support of trained foster parents and the Woodfords' treatment team, children and teens are offered guidance in navigating psychological, emotional, social, behavioral and learning challenges with the hope and intent for family reunification and life skills growth.
The Foster Care Case Manager provides oversight of foster families, develops treatment plans, comprehensive assessments, and conducts visits at foster homes bi-weekly.
The Case Manager supports the TFC program by serving the following :
- Provides Field Supervision of Foster Families
- Provides regular home visits to meet with family and consumer, both independently and together, to assess quality of services issued.
- Is on-call to families and is available to support them in case of emergency as directed.
- Provides for an annual evaluation of all of the families involved in providing services through our agency.
- Serves as a liaison between family and the differing agencies of oversight and funding, and advises the family and / or the Agency on the appropriate course of action.
- Serves as a liaison with other agencies involved in providing other types of services to the resident (public schools, rehabilitation agencies, consultants, etc.)
Opportunity Details
- Full-time, 40 hours per week.
- Monday through Friday , with occasional evening and on-call requirements.
- Hybrid / Remote Eligible!
- $44,000-$48,000 per year - commensurate with experience.
- $1,500 Sign-on bonus! - paid upon 6 months of employment.
Qualifications
- Licensed Social Worker (LSW) or must be eligible to become licensed. State of Maine licensure required.
- Bachelor's degree in Social Work or related field.
- At least (2) years' experience in the field.
- At least (1) year of professional experience working in the field of foster care / foster families.
- Possess a valid Driver's License and have use of a reliable, insured vehicle.
