Scope of Position

Lay Patient Navigator for GI medical and surgical oncology. A lay patient navigator is a trained professional who helps patients identify and remove healthcare system barriers, improving access to care.

The lay navigator acts as a link between patients and care providers to facilitate clinical and supportive care services.

The role is that of a single point of contact among referring providers, patients, and other clinical caregivers for the purpose of empowering patients in dealing with the disease and the treatment.

Position Summary

The Patient Navigator is responsible for assisting in guidance and coordination of oncology patients and their caregivers through the continuum of care.

  • Patient navigation : 1) is provided to an individual patient for a defined episode of cancer-related care; 2) has a defined endpoint;
  • 3) focuses on a certain set of health services to complete an episode of cancer-related care; 4) focuses on barriers to accessing care;

and 5) aims to reduce delays in the receipt of treatment services.

This includes development and assimilation of appropriate oncology educational materials for patients and their families.

The person in this position will work with a multidisciplinary team providing improved comprehensive oncology services to patients.

This position requires expertise to connect patients to appropriate community resources, identify and implement improvement processes, and the ability to identify and address patients’ concerns and barriers to care.

In addition, the individual must be able to discuss treatment issues and options with patients and discuss relevant issues associated with clinical trials participation.

Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers.

Responsible for other duties as assigned.

Minimum Education Required

Bachelor's degree or equivalent with at least 2 years’ experience in oncology, social work, nursing, community health education or related field.

Excellent customer service skills, good interpersonal skills including written and verbal communication skills and the ability to communicate with culturally diverse populations.

Must have the ability to maintain strict confidentiality, be self-directed with strong organizational skills, have the ability to complete multiple tasks simultaneously, and follow through so that each task is completed.

Functional computer skills including working knowledge and application of Microsoft Office.
