Position Summary : The Adoption Case Manager provides case management services to children and families in order to facilitate safety, permanency, and wellbeing.
Location : Olathe & Lawrence
Education : A bachelor’s or master’s degree in social work, community counseling, human development, child and family development, applied family and youth services, public health, health sciences, trauma studies, sociology / social services, substance abuse / addictions, early childhood education, or psychology from an accredited college / university.
Experience : Experience with children and family services preferred
Licensure / Certification : Current, valid Driver’s License and Auto Insurance required. Licensure by the Kansas BSRB is required
Essential Duties :
- Needs include, but are not limited to, developmental stages, trauma, poverty, gender, race, ethnicity and language.
- Actively seek to understand and support families’ lived experience and individual needs.
- Build and maintain strong engagement with clients, placement providers, DCF, coworkers and other community partners.
- Actively share relevant and appropriate information with family and involved professionals.
- Provide timely responses to all requests.
- Provide home and community-based services to clients.
- Services include, but are not limited to, observation of caregiver / child interaction, maintain both existing and developing connections, resource coordination, on-call crisis intervention, safety planning, skill building, transport clients and conduct drug-screens to ensure safety and well-being.
- Utilize information gathered through assessment process to advocate for the client’s best interest.
- Advocacy includes, but is not limited to, provide court testimony, attend school meetings and coordinate mental health services
- Complete required documentation in a timely and thorough manner.
- Documentation includes, but is not limited to, case logs, comprehensive assessments, court reports and social histories.
