The PACE Enrollment Specialist will play a crucial role in guiding potential participants through the enrollment process and ensuring they receive the necessary information and support to make informed decisions about their healthcare.
Responsibilities
Enrollment Guidance : Assist potential participants in understanding the PACE program, its benefits, eligibility criteria, and enrollment process.
Application Assistance : Provide support in completing enrollment applications accurately and efficiently, ensuring all required documentation is included.
Documentation Management : Maintain accurate records of enrollment applications, ensuring compliance with regulatory requirements and internal policies.
Communication : Serve as a primary point of contact for inquiries related to PACE enrollment, responding promptly and courteously to all inquiries via phone, email, or in-person meetings.
Education and Outreach : Conduct outreach activities to raise awareness about the PACE program in the community and collaborate with local organizations to reach potential participants.
Collaboration : Work closely with interdisciplinary team members, including healthcare professionals, social workers, and administrative staff, to facilitate a seamless enrollment process for participants.
Follow-Up : Conduct follow-up communication with potential participants to address any questions or concerns, and provide ongoing support throughout the enrollment process.
Compliance : Ensure adherence to all relevant regulations, including HIPAA (Health Insurance Portability and Accountability Act) and PACE program requirements.
Education & Training
Bachelor's degree in social work, healthcare administration, public health, or a related field preferred.
Previous experience in healthcare enrollment, preferably in a PACE program or similar setting.
Strong communication skills, both verbal and written, with the ability to explain complex information in a clear and concise manner.
Excellent organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously.
Empathy and sensitivity when interacting with elderly individuals and their families.
Proficiency in Microsoft Office Suite and experience with electronic health record systems preferred.
Ability to work independently as well as part of a team in a fast-paced environment.
