RN, Thrombosis Clinic 1.0 FTE

Position Summary :

Utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.

Supports multiple areas within a clinic setting with strong interpersonal skill and flexibility while maintaining patient confidentiality and protecting patient rights.

Resolves concerns or complaints regarding patient care or service.

Education / Experience :

• Graduated from an accredited school of nursing
• Associate degree or diploma required
• 1 year of prior experience in nursing preferred

Preferred :

Bachelor’s Degree in Nursing

Ambulatory Nursing Experience

Special Knowledge / Skills / Certifications / Licenses :

• Maintain a current Minnesota registered nurse’s licensure. WI licensure where applicable.
• BLS provider status in accordance with American Heart Association (AHA) guidelines
• Demonstrates knowledge of age-related growth and development necessary to provide appropriate care to the patient population served and ensure safety of patients.
• Possesses the qualities of strong critical thinking skills and ability to multi-task
• Ability to communicate effectively both orally and in writing, and maintain standards of clinical skills and professional ethics as established by the Nurse Practice Act
• Critical thinking, Excellent Interpersonal Skills, Fluent in EMR (EPIC) use
• Understanding of Nursing Practice Act and Scope of Practice for various roles

Additional Information :

We are looking for an experienced RN to join our thriving Anticoagulation team! This individual will support our Anticoagulation team by reaching out to patients, partnering with care teams and coordinating patient care.

In addition, this position withsupport our active Thrombosis clinic. Candidates should have strong nurse practice skills, demonstrate an ability to effectively deliver education, competently prioritize tasks and possess a desire to bring out the best in others.

The Park Nicollet Anticoagulation Clinic (ACC) provides centralized warfarin dosing performed by RN nurse clinicians and hospital pharmacists.

The ACC partners with primary care and specialty clinics to manage all areas of the patient’s anticoagulation care.The basis of the care model includes :

• Providing care in a centralized system using uniform and consistent methods and protocols in both the inpatient and outpatient setting
• Emphasis on patient education the out-patient setting
• Ongoing patient education and empowerment to patients to be active participants in their care
• Multidisciplinary team approach to care, including functioning as a resource for other health care professionals
