Job Description
The Vice President of Care Management is a key executive role responsible for leading and transforming the care management landscape within the organization.
This position will spearhead initiatives to deliver proactive care management, utilization review, outcomes optimization, episodic care management, and transitional care management.
The Vice President will be instrumental in developing a clear vision, establishing goals, and formulating a robust strategy to enhance and deliver proactive care.
Essential Functions
- Provide executive leadership to drive the care management vision and strategy across the organization.
- Develop and implement innovative care management programs that focus on patient-centered care and improved health outcomes.
- Lead the integration of teams from three distinct organizations to foster a coordinated approach to care management.
- Promote a team-based care philosophy to ensure a seamless and efficient patient experience.
- Optimize resource utilization and reduce waste through effective care management practices.
- Collaborate with stakeholders across the healthcare continuum to align goals and initiatives.
- Establish metrics and accountability systems to track and improve care management.
- Stays abreast of changes to the healthcare environment, care management and healthcare reimbursement innovations through local, regional and national networking.
- Supports best practice through policy standardization, appropriate data collection and analysis, IS support and clinical education collaboration.

Qualifications
Master’s Degree in Nursing, Social Work or Healthcare Administration. Degree must be obtained through an accredited institution.
Education is verified.
- Must hold and maintain a professional license in nursing or as an LCSW.
- Ten years of professional progressive experience in health care delivery or healthcare management
- and -
- Five years of experience leading case management, disease management or care coordination programs requiring knowledge of the current healthcare environment, healthcare regulatory matters and healthcare reimbursement.
- and -
- Experience in a role requiring strategic thinking and planning skills and the ability to develop proposals, pilots and projects from conception to implementation.
- and -
- Experience in directing other clinical areas, working closely with physicians, and developing clinical strategies, implementing operational efforts and measuring outcomes.
- and -
- Experience achieving results in both line management and matrix management environments.
- and -
- Experience using word processing, spreadsheet, database, internet, e-mail and scheduling applications.
- and -
- Experience in a role requiring effective verbal, written and interpersonal communication skills.
Preferred Qualifications
Professional experience working in an integrated delivery system that includes a health plan.

Experience evaluating technology solutions to support care management functions
and -
Experience with developing collaborative relationships with community organizations to address healthcare needs across the continuum
and -
Experience in an integrated healthcare delivery system where there is a strategy to move towards managing the health of populations.

- Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
- Frequent interactions with providers, colleagues, customers, patients / clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
- May have the same physical requirements as those of clinical or patient care jobs, when the leader takes clinical shifts.
- For roles requiring driving : Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
This is an exempt, full-time position with a compensation range of $327,475 $361,629, annually, and as determined by prior years of relevant experience.
With this position, you are eligible to participate in the Annual Pay for Performance (AP4P) Plan. This plan enables Intermountain Health to provide leaders with an additional performance compensation opportunity.
The AP4P award opportunities are calculated as a percentage of your base salary. Awards are paid out based on attainment of selected Board-approved goals.
This position is remote capable, with the expectation of up to 50% travel throughout the regions.
