Summary
The Family Support Coordinator is responsible forproviding homebased and communitybased family service coordinationservices.
They work to cultivate relationships between ourparticipants and their families to their local communities forengagement and services.
EssentialFunctions :
- Developindividualized Family Service Plans with the family they areassigned to in a timely manner
- Maintainsregular monthly contact with the child participant / family and makesfacetoface visits quarterly (at aminimum)
- Coordinates continuity of care andinvestigates alternative resources
- Assists withobtaining transportation for appointments related to the childsservice plan
- Assists family in identifyingneeds provides disability information coordinates various servicesand links family to service deliverysystems
- Enhances child / familys skills andabilities in procuring services to meet the childsneeds
- Promotes development of providerresources where services are incomplete ornonexistent
- Attends quarterly field supervisionmonthly staff meetings and weekly onehour individual supervisionmeetings with the Program Coordinator orDirector.
- Completes at least 8 hours of agencyorientation and 8 hours of target population training within fiveworking days of employment.
Completes 24 hours of FSC training and8 hours specific to Early Steps
Completes andsubmits all IFSP forms on time (5 days after themeeting)
Education / ExperienceRequirements :
- Bachelors or Masters Degree in a Human Servicerelated field.
- Experience working with childrenwith disabilities is preferable.
- Must showempathy and understanding toward and protect the confidentiality ofchildren / families.
Skills / Qualifications
- Excellent time and prioritymanagement
- Ability to collaborate withparticipants and their families as well ascolleagues
- Experience with Microsoft includingTeams Outlook SharePoint etc.
- Critical thinkingand problemsolving evidenced by pastexperiences
