Rising Ground is seeking a skilled Licensed Social Worker- Mental Health Coordinator (MHC) to oversee the specialized clinical team providing mental health assessments to students and their families at our three Bronx-based Community Schools, JHS22, PS195 and PS123.

The MHC will liaise with the school faculty of the three Community Schools and our in-house clinical team to provide interventions and participate in the development of individualized education plans to ensure that the clinical and psycho-educational needs of students are being met.

In addition, the MHC will work closely with the school-based support team at JHS22 in recommending or creating groups or other school-based activities beneficial to the school community.

Clinical intervention will be grounded in the clinical model of youth development, using a strengths-based approach. The Mental Health Coordinator will maintain clear and consistent communication with the Principal, Rising Ground leadership team, Assistant Principal, school faculty, school-based support team, students and their parents.

Responsibilities

Primary Duties : Community Schools :

Community Schools :

  • Coordinates clinical supports and services at three community schools providing weekly supervision to three school-based Mental Health Therapists
  • Oversees MSW intern program at three schools regarding their field placement, including providing clinical supervision

JHS22 :

  • Provides direct Individual / group counseling to students
  • Conducts initial student assessment and connects with parent / s to determine and arrange appropriate / adequate mental health services for students and / or their families.
  • Completes students’ psychosocial assessments, treatment plans, and case notes to ensure continuity of care
  • Provides crisis counseling to students
  • Conducts home visits as needed; provides linkage and referrals to support services within the school and within the community
  • Supports School Guidance Counselors with case conferencing and help create beneficial school wide campaigns
  • Assesses school needs to develop workshops for students, parents, and / or teachers
  • Provides school staff with psychoeducation about adolescent development and mental health related topics
  • Facilitates student workshops on various social / emotional topics, including stress management, healthy relationships, impulse control, grief, etc.
  • Is an active member of school interdisciplinary teams
  • Provides parent outreach, community networking and referrals for outside services, as needed
  • Documents all client services in electronic health record (EHR-MyEvolve), to include OMH and Daily Services Log reporting
  • Interfaces and collaborates with relevant Rising Ground programs and / or outside providers
  • Participates in and conducts supervision; 1 : 1 and group, may be over the phone or in person
  • Participates in school-based or Rising Ground meetings related to MH services
  • Other duties as assigned

Qualifications

  • LMSW required, with at least 3 years of LMSW experience
  • SiFi Certification must be obtained within 3 months to retain employment
  • Strong engagement skills
  • Strong communication, organizational, and behavioral management skills
  • Ability to lead a mental health team of up to 10 interns
  • Experience working with children and adolescents in a school setting preferred
  • Experience with groups
  • Passion for incorporating reflective practice around issues of race, cultural diversity and intersectionality into clinical work
  • Sensitivity to the cultural diversity of the population
  • Self-motivated, self-starter, ability to work independently
  • Excellent organizational skills, ability to multitask
  • Strong communication skills both written and verbal
  • Ability and willingness to travel throughout NYC and conduct home visits, if needed
  • Knowledge of DSM TR Criteria
  • Bilingual (Spanish and English) preferred

Salary $90,000 / Annually
