Mountain View Hospital is looking for a HIM Clerk to join our team!
JOB SUMMARY :
Reviews medical records after discharge for completeness in compliance with the hospitals bylaws, federal and state requirements.
Assures that patient’s abstracts are correctly entered into the chart deficiency system. Monitors accounts being admitted daily for duplication of medical records numbers, along with checking patients discharge status when assembling the chart.
Ensures that all transcribed reports are filed in a timely manner in the charts. Communicates with physicians about delinquent reports.
Accountable for the accurate transcription of dictated reports. Assures the completion of incomplete medical records within the time frame as specified by the medical staff rules and regulations.
Is familiar with requirements of the state guidelines of time frames for processing of the charts such as the History and Physical and Operatives report time frames allowed.
Also knows Medical Record content, maintenance and retention of records.

DUTIES AND RESPONSIBILITIES :
- Collects all discharged patient charts from the patient care units on a daily basis.
- Collects loose filling for those discharged charts and inserts such filing in proper charts.
- Assembles discharged patient charts according to assembly list.
- Places assembled chart in proper folder according to medical record number.
- Analyzes medical records of discharged patients for completeness and accuracy according to department and hospital policies and procedures.
- Assists in retrieving charts for physician completion.
- Answers incoming calls to the department in a professional, timely manner.
- Forwards all analyzed charts to the coder.
- Pulls delinquency charts according to deficiency list.
- Files or scans all charts back to main file.
- Makes and attaches medical record numbers for new charts.
- Communicates appropriately and clearly to the supervisor, nurse managers, co-workers and physicians.
- Handles correspondence from insurance companies and attorneys. Obtaining requested medical records to fill those requests.
- Completes annual education requirements.
- Must be able to keep up with work load be able to process a minimum of 25 charts per day.
- Reports to work on time and as scheduled, completes work within designated time. Reviews charts thoroughly to ascertain all diagnosis / procedures.
- Performs quality review on records scanned in Evident.
- Reconciles master list with the computer generated in house list on a daily basis. Must check admit and discharge dates for accuracy in the mainframe computer.
- Requests charts daily from storage for next day procedures.
- Files and clears all charts in the dictation room by the end of shift on a daily basis.
- Responsible for knowing hospital policies and being familiar with hospital services that are made.
- Demonstrates an ability to be flexible, organized and function well in stressful situations.
- Treats patients and their families with respect and dignity; ensures confidentiality of patients’ records.
- Maintains a good working relationship both within the department and with other departments.
- Participates in educational programs, in-service meetings and completes annual educational requirements.
- Attends all other meetings as required.
- Maintains patient confidentiality at all times.
- Accepts and completes other duties as assigned or requested.
- Retrieves transcribed reports check for errors on transcription notifies company if error is found, has transcription record fixed when updated.
- Assured history and physical is on each patients chart prior to surgery.
- Assures operative reports are dictated immediately after surgery.
- Contacts physician for delinquent reports, if necessary.
- Processes physician and other department staff requests in a timely manner.
- Other duties as assigned.

Qualifications
Education / Certification : High school diploma or general education degree (GED).
Experience : One to three months related experience and / or training; or equivalent combination of education and experience.
Equipment / Technology : Ability to use hospital information system, and operate basic computer functions and software.
Language / Communication : Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals along with the ability to write routine reports and correspondence, ability to speak effectively before groups of customers or employees of the organization.
Mathematical Skills : Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. .
Mental Capabilities : Ability to apply common sense to carry out instructions furnished in written, oral, or diagram form along with the ability to deal with problems involving several concrete variables in standardized situations.
Interpersonal : Ability to work well with others.
